Birth certificates​
Social Security cards
​Photo Identification for household members 18 & over
These documents are REQUIRED for all household members and must be submitted before your application can be completed!
PLEASE BE AWARE
If you submit an application without all the above documentation we will not be able to accept your application.
Please be sure you have all documents before submitting!

If you are pregnant, please provide proof of pregnancy.

If you are not an American citizen, please provide your Alien card.
​
If you receive SSA or SSI, please provide a statement of benefits.
​
If you receive child support, please provide a current printout of the last six months of payments.
​
If you have a checking and or savings account, please provide a bank statement.
​
If you are married, please provide your marriage certificate.
